Black Mark

Meaning: A bad report or record for doing something wrong.  It is usually considered a warning.  More than one black mark can result in some sort of punishment. 
How to use in a sentence: The teacher gave Mary a black mark in her mark book because she didn't do her homework.  If she got three black marks, the teacher would give her a detention or phone her parents.
